Purpose:

To report the outcomes of a novel approach to the 2-stage surgical handling of coexisting visually significant cataracts and corneal opacities

Methods:
|
A retrospective investigation of eyes with cataract and corneal opacities that were surgically treated with cataract and simultaneous penetrating keratoplasty, trailed by secondary IOL implantation after evacuation of corneal sutures, was directed. The parameters utilized for appraisal were the accompanying: mean level of graft endothelial cell misfortune after IOL implantation, deviation of the post surgically mean spherical equivalent from the mean uncorrected distance visual acuity (UDVA) and target refraction

Results:
|
nine eyes were incorporated into the examination. The mean UDVA was 1.14 ± 0.56, and the mean corrected distance visual acuity was 1.56 ± 0.42. The mean interim between the 2 surgical intercessions was 6.3 ± 1.92 months. Just before secondary IOL implantation, the mean endothelial cell density was 2098 ± 411 cells. The mean level of corneal endothelial cell loss was 6.3% at 3 months after IOL implantation. Before IOL implantation, the mean spherical equivalent was +10.75 ± 2.38 D. After IOL implantation, the mean spherical equivalent better to -0.17 ± 0.83 D at 3 months. The mean UCVA improved to 0.24 ± 0.11 (P = 0.017), whereas the mean corrected distance visual acuity better to 0.17 ± 0.28 at 3 months. All grafted corneas kept up their clarity until the point when the last follow-up visits.

Conclusion:
|
Postponing IOL implantation a few months after cataract extraction and simultaneous penetrating keratoplasty negligibly affects the corneal graft endothelium and accomplishes near postsurgery target refraction with significant enhancement in UDVA.
