Purpose:
Pressure ulcer is skin break¬down caused by excessive or prolong stress exerted to a location due to patient position or medical devices applied for diagnostic or therapeutic purposes. The aims of this study are to present 4 cases with forehead and eyebrow pressure necrosis after cosmetic and functional upper face procedures, review pathophysiology and etiology of pressure necrosis, and provide a guideline to avoid and manage this complication after upper facial procedures.
Methods:
small case series and literature review.
Results:
This case series reports forehead pressure necrosis following cosmetic forehead lift (3 patients) and eyebrow dermoid cyst excision (1 patient) in otherwise healthy subjects.
Conclusion:
There are 3 main factors contributing in the pathophysiology of pressure necrosis: pressure itself, shear forces and reperfusion injuries. Tightness of dressing and the frequency of skin examination beneath dressing are two main area of focus in prevention of pressure injuries. Treatment of pressure ulcers focuses on different types of dressing, topical medications and debridement methods based on ulcer characteristics, but there is no consensus regarding the treatment most likely to heal pressure ulcers.
